Is this form HIPAA compliant?
Yes. This form is submitted through a format that is encrypted and HIPAA compliant.
What information do I need to submit in the referral?
While all fields are optional, the key information to include is your name and contact information, as well as the patient’s name and one way to contact them (phone or email).
What happens after I submit the form?
A notifcation will be sent to our Admin team. They will contact the patient within the next business day to schedule a free, 15 minute consultation and start their care.
Can I just send the patient’s chart and records instead of explaining the information on the form?
Yes! You can always fax (703-661-9463) or email (admin@gipsychology.com) supplemental patient information to the Admin team. The information will be uploaded to their chart for their clinician to review to ensure well rounded care.
What is Auto-reporting?
Auto-reporting is the automatic process of our team sending the treatment plan following the intake and the termination note following termination to you, with patient consent. If this is something that you are interested in, please ensure to provide the best way to our Admin team to provide this information.
What if the patient I refer does not consent to the Auto-reporting?
Unfortunately, without proper consent, we are unable to provide such updates under the patient privacy laws.
Is the Auto-reporting the only update I will receive from my patient’s clinician?
Our clinicians understand and emphasize the importance of care team collaboration. With the proper consent, our clinician will coordinate care collaboration throughout the patient’s time with GI Psychology.
